  • Position Summary

    Position Summary:  Responsible for the collaborative process of assessment, planning, facilitation and advocacy for options and services to meet health care and bereavement/end of life needs through communication and available resources to promote quality cost effective outcomes.

     

    Responsibilities

    Essential Functions:

    • Collects and assesses initial and on-going data about the psychosocial status, cultural influences and belief or value system of the patient and family.
    • Assists in initiating and/or revising a plan of care process for end of life issues.
    • Provides patient education, evaluation of patient response, utilization reviews and quality assessments that reflect the plan of the entire health care team.
    • Carries out interventions safely and in compliance with policy to facilitate achievement of expected outcomes.  Follows the plan of care regarding end of life issues, reports uncontrolled physical symptoms, addresses psychosocial needs, grief and bereavement needs.
    • Provides patient education by explaining plan of care and available resources at a level the patient can understand, addresses patient’s questions and concerns, verifies patient informed consent when indicated.
    • Evaluates patient response based upon expected outcomes, Documents and communicates all required components of patient care.

    Qualifications

    Education/Experience: Graduate of an accredited school of social workers; hospice and palliative care experience preferred; knowledge of general social worker theory and practice required.

    Certificates and Licenses: Masters degree in social work (MSW) with current licensure in the State of Kansas required; BLS certification preferred; valid Kansas drivers license with an acceptable driving record and must be insurable through the medical center’s insurance carrier (may be required to use personal vehicle); and additional certification may be required for certain patient care settings or populations.
